Front Porch Family Practice was started by a family physician that strives to provide high value health care within a family comfortable environment. Our carefully curated team of providers are here to give you the treatment you deserve. Dull and cold hospital waiting rooms are a thing of the past.

FPFP is a membership based health care service that provides affordable membership options for your healthcare needs. We do not accept insurance. If you have health insurance we will provide you with a receipt that you can submit to your insurance company for reimbursement.

Membership includes:

• Annual wellness visits

• Urgent /same day virtual or in-person visits

• 24 hr access for urgent health care advice

• Access to home visits

• Discounted prices on procedures, infusions and extra services

• Referrals to specialists and for imaging at affordable cash pay prices

 

*For patients with more complex medical needs, we will create personalized membership plans

 

How do I sign up?

 

Book an office visit/consultation for $150 

We will assess your urgent or ongoing medical needs.

If our practice is a good fit for you, you can sign up for a membership.

 

*$150 will be applied to your membership fee.

Do I have to have a membership to be seen?

No! There is an a-la-carte price for all services provided below:

 

• Wellness Visits

• Insurance Forms 

• Urgent Care

• Women’s Health (pap smears, birth control, etc)

• Counseling

• Sutures

• Joint Injections

• Skin Procedures
